My dad was transferred from a brief two week stay in the hospital to this facility for “elderly dementia care”. He originally was admitted into the hospital in order to regulate his medication and calm him down, nothing to do with his actual physical health which was never a problem. Due to him being in good physical health the hospital had to discharge him into this facility where they gladly accepted him and assured us that he would be taken care of. That was far from the truth. During multiple unannounced visits we witnessed multiple incidents of improper care. Missed medications, dehydration, multiple leisure’s and bed sores, lost or stolen personal property, and improper hygiene. We would find him wearing other patients clothes and dying of thirst. At one point he had sustained a torso injury which we reported and no one bothered to follow up with us on his medical evaluation or if any took place. He was losing lots of weight every week to the point that his grandchildren could barely bare looking at him. The last straw was them sending him to the hospital while very ill where he eventually died 2 days later and no one even bothered to call and let us know he had been transferred to the ER. This place shouldn’t be in the business of caring for the most vulnerable. Keep your loved ones away from this place. If I could give it zero stars I would.

This place is awful, and even the staff seems to agree. The African-American lady at the front desk was unprofessional, as were the rest of the staff. I arrived at 3 PM to check out the facility for myself. They let me walk in without verifying if I belonged there, and there was no one around. Those I did see were either on their phones or neglecting patients. I had planned to place my father here for six months, but after seeing this, I walked right out. The reviews are right—do not trust this facility with your loved ones. If you have family here, make sure to check on them.

A few years ago, I gave this place five stars, but it deserves zero. My grandma wasn’t given her medication properly, causing more problems. There’s proof in reviews, like someone walking in without showing ID. Every time I visit, employees are on their phones, and elderly people are left standing in hallways. The rehab room is always closed. This place is a mess—don’t put your loved ones here. Any positive reviews are likely paid or written by staff, who won’t speak negatively about their own workplace.

I have been a resident of this facility for 3 years and I have no complaints. The staff have always been great and attentive to my needs. My favorite person is Steven in the activities department. From day one he made me feel welcome and wanted. House keeping keeps the place up to standards with the upkeep of the building. I have toured other places but this place feels like home.

I love the place I'm in. It's got me a lot of improvement on my health and body. Especially the activities department. They really show that they care. They check on you when you're in the hospital and make you feel like family. They deserve a 10 star because 5 isn't enough. The folks in there are super. I just love them .

This place is horrific. I had problems from the very beginning. I was lied to about my mother's available room by the liason Tonya. During my mother's stay, she was not given snacks at her request, was not turned and her wound on her bottom opened up. They were not even aware she had this wound but if she was turned- they would have seen it clearly. She was put on the ground, yes the ground to have her diaper changed. I had to request a different blanket one day because they covered her up in a blanket that had feces on it. I tried for 3 days to get my mother transferred to a much better facility. They refused. I spoke with marlisa Thompson, the director. Long story short she called the police on me, so I called the police on her. She proceeded to lie about my mother's care, told the police I threatened her and her staff all while she is approximately 2 inches from my face yelling at me.
Her staff, social services office laughed at me. I told Ms Thompson I will be calling adult protective services. I had to have my mother transported to the er in order for her to leave. I waited in my mother's room until transport came. As soon as my mother realized she was getting out of there, she cried hysterically. Keep in mind I am my mother's poa and health surrogate. I do not want any son or daughter to go through such a horrible situation. Do not take your parents here. It truly was a nightmare. Now my mother is constantly worried she will be put on the floor. She lost her dignity.

My mom was there for 21days. Sent home with no after care plan. I stayed with her and within 36 hours of being home had to call 911. She was admitted to icu with a UT I and ecoli caused by the rehab not taking her to the bathroom and leaving her in soiled briefs for hours. They almost killed my mother. She was in hospital for a week and now at another rehab. Don't send your worst enemy to this death trap. I'm still considering filing suit
