Moving to a new state and finding a trustworthy doctor can be daunting, but I feel incredibly fortunate to have found Dr. Colin Chaloupecky and his team. From the moment I walked into the office, I could tell I was in excellent hands.

Dr. Chaloupecky has an incredible bedside manner. He truly listens, never makes you feel rushed, and takes the time to explain things thoroughly. During my first visit, he noticed I was on cholesterol medication and immediately expressed concern, saying I was too young to be on it. Instead of just going with the status quo, he suggested we work towards getting me off it. His sincerity and empathy were evident from that moment, and it was clear he genuinely cares about his patients’ long-term well-being.

The office staff, especially Joan and Sandy, went above and beyond to ensure everything was coordinated seamlessly. They made sure my bloodwork, imaging, and even my specialist referrals were all in-network, saving me a ton of time and effort. They truly take pride in their work and were a joy to interact with.

The intake nurses were just as wonderful—smiling, kind, and clearly happy to be there. It’s rare to find an entire team that genuinely cares about patients at every step. On top of that, the facility is beautiful, which adds to the comfort of the experience.

I’m confident I’ve found not only a great doctor but a fantastic office for my entire family. Finding a high-quality primary care physician with a staff you can trust and enjoy working with isn’t easy, but this team has it all. I highly recommend them to anyone new to the area or looking for compassionate, top-tier care.

For the last few months, I have been treated weekly by Cristian, the athletic trainer at CFH&SM, for a stubborn shoulder issue. I have experienced measurable results with his help. What makes physical therapy with him more productive and effective than at other therapy centers is that he focuses on one patient at a time. He doesn’t juggle multiple patients simultaneously, but rather, works one on one with each patient. That allows him to get to know each patient and their individual issues better, and he’s creative at trying different methods to restore health and mobility.

If you need physical therapy, I highly recommend Cristian, and encourage patients to receive well-care for their family at this practice.

This office is always a pain; never caters to the customer/patient. I advised the practice that I would be switching practices, and they closed out my patient portal (access to all of "my" records). I needed an invoice/diagnosis of my last visit for my insurance company, and they said (JOAN) that I would need to drive the 23 miles to the practice to get it, and refused to email it, fax it, or even send it in the mail. SO glad I'm out of there!

As a woman, I prefer lady health practitioners. The practitioner I was seeing moved out of state so only male practitioners remain. I’ve asked several times over the past few months about when/if women practitioners will be on staff soon & they tell me the same thing: “yes but we don’t know when they’ll start”. You can get away with saying that the first time but months later is problematic as it erodes trust. They’ve either hired someone & know their start date or they haven’t. It’s now very clear they haven’t. Moving on to another practice that employs women practitioners & doesn’t lie to their patients.

Amazing team and atmosphere! It's a place where you truly feel valued as a patient, not like just another number. Highly recommend!

Rude older receptionist that didn't know what types of services they offered. Asked about a specific type of manipulation offered by most physical therapists and told me that I couldn't request injections When I said that if she wasn't sure what they offered, I wasn't sure about booking with them and she hung up on me. If I were the manager of the practice, I think I'd want to know that potential clients are being treated this way.

I have been a patient for about 10 years. Everyone is wonderful and very professional. I would like to give a special thanks to Dr Martin. I appreciate that little extra time you spent with me, explaining things more clearly for my understanding. It was a very positive experience. God bless you all.

I saw Dr. Robert Martin for my yearly physical as a new patient. He was very thorough and engaging and I felt very confident in his ability to assess my needs as a patient. Joan was a delight and scheduling an appointment with her was a breeze. She was very helpful with setting up my patient portal!
